Referrals and Admissions

Homewood has a national mandate to deliver highly specialized care to all Canadians. We accept referrals from throughout the country and around the world.

The admission process is similar for all patients. Costs and payments will vary depending on the type of accommodation and the approval of provincial health ministries.

Step-by-Step Process to Admission

  1. The admission process begins when a Referral Form from a health care professional is received by the Homewood Admitting Department.
  2. Upon receipt of the referral, Admitting sends a letter of acknowledgment and an information package to the prospective patient and the referral source. At this time, a Pre-Admission Information Form is to be completed and returned to the Homewood Admitting Department. Note: additional forms are required for Homewood’s Eating Disorders Program.
  3. Upon receipt of the Referral Form and Pre-Admission Information Form, Homewood reviews the needs of the prospective patient to determine if the programs offered at Homewood provide a suitable match.
  4. Once the referral is approved, Homewood’s Admitting contacts the prospective patient to discuss accommodation requests and confirm financial arrangements.
  5. Patient will be provided with an Admitting Case Manager, who can provide waitlist information (if necessary) and details about specific program requirements.
  6. When an admission date is set, patient and his/her health care provider will be contacted with all necessary admission details.

Please note: admission days are determined by the specific program, and appointment times for admissions are provided by the Admitting Department. All admission appointments occur between 8:00 and 9:30 a.m. Monday through Friday (excluding holidays.)
